We have a variety of on-line forms that will help expedite your pet’s visit with us. We would appreciate if you either email us at info@bigcreekpet.com or fax (440-243-9681) the completed form back to us prior to your pet’s visit. We check our emails typically early morning and then again about 3 pm. We will call or email you to confirm receipt of any information you send us.

If your pet is getting his or her teeth cleaned, we would like to know whether you want us to pull any bad teeth or if you would like us to only clean the teeth and be referred to a veterinary dental expert for advanced procedures, such as, root canals and restorative care.
